We take a holistic approach—supporting not just physical health, but also emotional, spiritual, and practical well-being.

Our nurses are at the heart of what we do. They are more than clinical experts—they are advocates, guides, and steady sources of support. With empathy and clarity, they help patients and families navigate the hospice journey, ensuring comfort and peace of mind at every stage. A Registered Nurse (RN) is available 24 hours a day, 7 days a week, with Licensed Practical Nurses (LPNs) providing additional care and support as needed.

Comprehensive Hospice Services

Our interdisciplinary team includes a wide range of professionals dedicated to caring for both patients and their loved ones:

  • Physicians & Medical Director – Collaborate with your primary care physician to oversee care
  • Nursing Team (RNs & LPNs) – Coordinate care, provide clinical support, and ensure comfort
  • Social Workers – Offer counseling, emotional support, and assistance with social, financial, and community resources
  • Dietitian – Provides guidance on nutrition and hydration and align care with the patient’s needs and preferences
  • Spiritual Counselors (Chaplains) – Provide spiritual guidance and coordinate support with community clergy if desired
  • Hospice Aides – Assist with personal care such as bathing, dressing, grooming, and mobility
  • Therapists – Physical, occupational, massage and speech therapy services when appropriate and physician-ordered
  • Nurse Practitioner – Provides additional clinical support and in-person visits as needed
  • Bereavement Specialists – Support patients and families through grief and loss
  • Volunteers – Offer companionship, assistance, and support for patients and families
